Audiology
How to Apply
KU uses CollegeNet for processing online applications. The application fee is $60. Note: the AuD application uses the KU Medical Center application system and a different link than for KU Lawrence campus. Please ensure the application is for the AuD degree (not the PhD in audiology). It will be helpful to have the following information ready before starting the online application:
GPA in the major calculator: Using your degree plan as a guide, include all courses in your undergraduate major within the application's electronic GPA calculator. Do not include courses required by the College (e.g., history, college algebra, etc.) UNLESS they were required by your degree plan. If a course was required in your undergraduate degree plan but was not offered in your department, please include the course (e.g., linguistic, statistics, research methods). The GPA calculator is only to calculate the GPA in your undergraduate major, whether it be in CSD or another field. If you double majored, choose the degree more closely related to SLP or CSD.
Personal statement questionnaire: Applicants will be prompted to answer the following questions on the application concerning career goals and interests:
What are your professional plans after graduate school?
What are your research interests and goals? Why?
What area(s) of clinical practice interest you the most? Why?
List the faculty member(s) with whom you would like to work.
Anything else you would like to add to strengthen your application?
GRE scores: Applicants must request official scores on the Graduate Record Examination be sent to KU Medical Center using institution code 6895 and audiology program code 0602. The department office will upload the scores into the student's open application. It is strongly recommended GRE scores be requested no later than Dec. 31 to meet the Jan. 15 application deadline. Note: Once GRE scores have been sent to KU, they can take up to two days to connect to your application. If you do not see your scores appearing in your application checklist, please do not contact the office until a week after you have sent your scores.
Curriculum vitae or résumé: Provide a 1- or 2-page PDF document of work and academic history including professional leadership contributions as well as volunteer activities.
English language proficiency: All applicants whose native language is not English must demonstrate English language proficiency through either the TOEFL or IELTS testing systems. The test must have been taken within two (2) years of the first semester of enrollment. Results must be provided directly to KU by the testing service using ETS school code 6895. Photocopies of scores will not be accepted.
References: A reference provides a professional recommendation on your behalf. It is preferred that college instructors provide these recommendations, but past supervisors during employment or volunteer positions are also acceptable. The recommendations may not be obtained from family members, friends, etc.
Applicants will enter the names and contact information (including email addresses) of three (3) individuals into the online application. The system will then prompt each person to submit a reference letter and the graduate letter of recommendation form.
Make sure each reference is willing to submit this electronically, and inform him or her that they will be receiving an email from "KU Medical Center " requesting them to submit a reference for you online.
Transcripts: Please provide an unofficial transcript from every institution from which a degree has been received. In addition, ALL prerequisite courses, even if acquired apart from the degree, must be documented on unofficial transcripts. Note: printing a page from a website does not meet this requirement. All transcripts must be uploaded by the student with his/her online application for the application to be considered complete. Transcripts provided to KU in any other way will not be considered part of the application.

Evaluation is based principally on academic, research and clinical achievement and potential, including evidence from extracurricular activities. Please note the admissions committee may contact applicants for a potential interview after the January 15 deadline if being considered for the program.
Applicants are notified of application status by April 1 for fall program start.
Accepted students deciding to enter the program will request an official transcript during orientation to provide documentation of successful completion of an undergraduate degree.
International transcripts must be evaluated by World Education Services or Educational Credential Evaluators. The evaluation must include course-by-course evaluation, grade evaluation, and indicate the degree earned in U.S. terms (bachelors, masters, doctorate, etc). An official copy of the evaluation is required.

IMPORTANT: Before beginning an application, please contact a KU advisor in CLS to review prerequisite courses. Before you start the online application
It will be helpful if you have the following information ready before starting the online application:
Personal Goals Statement: Please explain why you are interested in pursuing this profession, experiences or activities that have helped you prepare to enter a CLS program, your personal and professional goals, why you feel you would be successful in our program, etc.

A reference provides a professional recommendation on your behalf. At least two (2) of the references must be science instructors, preferably laboratory instructors (graduate teaching assistants are acceptable). The third reference may be provided by an employer or an instructor in an area other than science. The recommendations may not be obtained from family members, friends, etc. Make sure each reference is willing to submit this electronically, and inform him or her that they will be receiving an email from “KU Medical Center ” requesting them to submit a reference for you online.
Course Work Plan: Have ready a list of all courses in which you are currently enrolled, and in which you plan to enroll to complete the prerequisites. All required course work must be complete prior to the start date of our program. You will need to list for each course the course number, title, credit Hours, and institution attended.
Shadow Documentation: Please review the options for fulfilling this item on the eligibilty and requirements page. The shadow documentation formcan be printed, completed, scanned, and uploaded directly with the online application. The shadow documentation form may also be emailed to the admission committee at CLStranscripts@kumc.edu.
Employment History: We are interested in paid employment positions held while attending college classes. Please provide the approximate number of hours per week worked per semester, the number of credit hours enrolled while employed, each semester's GPA, the name of the employer, and job responsibilities. Applicants are not penalized if they have not worked while attending college.
Review the program's technical standards and requirements, which describes the abilities and expectations of all students in the KU Clinical Laboratory Science program. Before submitting the application, students must acknowledge having read this document.

This page is a guide to the application process. Please verify all requirements are met before beginning the application process. Applications submitted by February 1 receive priority status. Applications may be accepted after February 1 until all seats in the program have been filled for the next fall class.
Before starting the online application
It will be helpful to have the following information ready before starting the online application:
Prerequisite Review: Verify all prerequisite course work will be completed by the fall semester start of the program.
Letter of Intent: Please write a 1-2 page letter, double-spaced, using a common font (e.g. Arial, Times New Roman), and save it as a Word, PDF, or rich-text format document to upload with the application.
The letter should address the following:
1) Describe how you learned about the profession of health information management, and why you decided it would be your chosen pathway.
2) Discuss opportunities and challenges you see on the horizon for HIM professionals.
3) Discuss your career goals and how you see that a bachelor's degree will support your goals.
Resume: Indicate your education and work background, honors, memberships in organizations, service to the community outside of school-related activities, and any other information that you might deem important for the committee to consider.
References: Applicants will enter the names and contact information (including email addresses) of three (3) individuals into the online application. The system will then prompt each person to submit the reference online.
A reference provides a professional recommendation on your behalf. One reference should be from an instructor/professor. A second should be from a manager or supervisor of a work/volunteer organization. The recommendations may not be obtained from family members, friends, etc. Make sure each reference is willing to submit this electronically, and inform him or her that they will be receiving an email from “KU Medical Center ” requesting them to submit a reference for you online.

English Language Proficiency: All applicants whose native language is not English must demonstrate English language proficiency through either the TOEFL or IELTS testing systems. The test must have been taken within two (2) years of the first semester of enrollment. Results must be provided directly to KU by the testing service using ETS school code 6895. Computer Requirements: All students in the program are required to possess a laptop or tablet which meets the minimum hardware and software requirements. Students are responsible for having the laptop or tablet in class in order to access course materials and take exams. The requirements will be sent to students upon acceptance in the program.
International Students:

After Submitting the Application
Admission decisions for completed applications are made during the spring semester. Late applications will be reviewed after the spring semester. Once all application materials are received, selected applicants may be called to schedule an interview with faculty of the department.
Upon acceptance into the program, students will be sent an enrollment packet consisting of background check instructions and information about computer requirements.
